Funded by the ACT Health Directorate, Home and Community Care Programme (HACC), The Community Care Program offers a range of support activities for people who are frail aged and those living with a disability and their carers living independently in our community and is focused on promoting the independence and community involvement of program participants. Support provided is a mix of tailored in home support in addition to social groups.
