Bridges is funded by the ACT Health Directorate, Home and Community Care Program (HACC) for people with a disability, older people and carers who are socially isolated or at risk of becoming isolated. It aims to provide support to increase community access so that an individual can connect with their chosen activity/community group. Support may be in the form of assistance with transport, life skills training, accompanying individuals to a chosen activity or addressing other more specific needs such as translation/cultural support. Anyone can make a referral to Bridges through the intake worker at Belconnen Community Service who will organise an assessment with the Bridges coordinator. Eligibility People who are carers, older people and/or people who have a disability and the ability to develop or enhance skills to become involved in mainstream community activities thus reducing social isolation. Bridges conducts some recreational and life skills training programs in a group setting at various times through the week during school terms. Term fees apply.
