




This year the Community Care Program has provided information, short term brokerage, case management, shopping support and social groups to people who are perceived as frail aged, younger adults with a disability and their carers living in the Belconnen region.
Our Case Management has provided support to numerous clients with complex needs, who typically require support from more than one agency. Case Managers have worked in collaboration with participants, external agencies and health professionals to develop goals and care plans that promote independence and provide opportunities for individuals with complex care needs to remain living at home for as long as possible.

We have also provided many referrals to external organisations for long term assistance with the above services.
Our one- off garden tidy up to maximise safety has increased considerably this year and the provision of services or equipment that encourages and promotes independence, we have also seen rise. Some examples are: purchasing curtains for a participant who has none during the cold winter months; providing skips for participants to enable them to clean up their properties; purchasing a new mattress and assisted individuals who were in financial hardship contributing towards essential services such as gas and electricity records so accounts were not disconnected; and, connecting participants with financial services to enhance their budgeting skills.
This year we have provided shopping assistance to individuals some with vision impairments or memory issues. This support is provided on a fortnightly basis and further supports participants to remain in their own home.
The Community Care program facilitates two programs that provide social interaction for people living independently within the community. Both programs offer guest speakers, activities, games, outings and exercises to increase strength and mobility for participants.
